Test anxiety may be the cause of poor test performance among students. There can be many physiological responses in the body that lead to an anxiety attack. One is the body’s response to stress, which causes the brain to release hormones that stimulate central and peripheral nervous system responses. The hypothalamus-pituitary-adrenal axis (HPA) is the one that responds to stress, causing an increase in cortisol in the blood, a glucocorticoid (GC) hormone that affects nerve responses. Concerns during testing include the style of information presented, prior knowledge of the topic, level of emotion, or how well the student can retain knowledge. The mental state of the student is important and can cause a strong activation of the HPA axis during stress during the tests, worsening the cortisol levels and the nervous response. This review discusses the potential involvement of HPA stressors in individual performance during testing and evaluation.

There is an increasing emphasis on assessment, especially in math and reading achievement, for national development, economic competitiveness, and the promotion of independence [1]. However, many students have anxiety because of the stress caused by such competition [2, 3]. Some students benefit from accommodations that can reduce stress and anxiety [4]. According to Sawker-Miller, test anxiety refers to mental, psychological, and/or behavioral symptoms of anxiety before or during the test-taking situation that can interfere with the test process [5]. Many students with test anxiety struggle to stay focused on the test, which can lead to poor test performance [6]. Test anxiety is one of the most difficult issues in school, yet it has not been properly addressed [7, 8, 9]. This particularly affects those who struggle academically or are known to have a learning disability (LD).

Test anxiety and general anxiety disorders are related to certain types of mental behavior described under the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). Test anxiety is an important factor that contributes to lower academic achievement and test scores, as previous studies have shown that test anxiety is associated with lower academic achievement among students [10, 11, 12, 13]. Pekrun et al. showed that test anxiety reduces working memory and causes impaired performance with more complex or difficult tasks [14]. This may be related to the physical stress caused by the psychological effects of test anxiety. There are accommodations, discussed below, for students to understand stress and take action to help cope with these situations during the exam.

Test anxiety is an issue among all students but it is more evident in grades 3 to 12 and can be a cause of poor performance in some students. Inadequate tests and poorly understood physiological responses during testing (eg, cortisol and stress hormones) can be issues that exacerbate test anxiety. Poor test performance can affect a student as an adult and reduce a person’s ability to find a job and earn a living. About a third of all students are predicted to experience test anxiety, regardless of their ability level [15]. Many stressors can lead to test anxiety, affecting the way a student receives, retains and understands information. In this review, we discuss the physiological response of the human brain and how stress signals molecularly disrupt human cognitive abilities, which can affect anxiety and depression during experimental conditions. We also discuss how accommodation can help prevent stressful situations and describe other methods that can help improve test anxiety.
