The veins in this system send blood to the liver rather than directly to the heart. The portal vein is the main blood vessel in this system. To understand your portal vein and its function, it’s helpful to learn a little about your portal venous system.

Numerous smaller veins in the portal venous system collect blood from all of these organs and deliver it to the portal vein. You can think of your portal vein as a pit lane on a racetrack. This leads to the liver, which serves as a stopping point for the blood to receive necessary maintenance.

By the time the blood reaches the portal vein, it is full of nutrients that need to be processed before your body can use them. This blood also contains toxins or harmful substances that your body does not need. Your liver “tunes” your blood by converting nutrients into forms that your body can use immediately or store. It also removes toxins from the blood.

After this adjustment, your blood is ready to return to your heart. Thus, it leaves the liver and goes to the inferior vena cava, which takes blood to the upper right chamber of the heart (right atrium).

All other veins in the hepatic portal system converge (join the pathways) and lead to the portal vein. Therefore, it needs to be healthy and functioning at its best for the entire system to function as it should.

The portal vein begins just behind the neck of the pancreas and in front of the inferior vena cava. It extends to the porta hepatis (hilum of the liver). This is an opening in the liver that allows blood vessels and other structures to enter and leave the liver.

The portal vein forms at the point where the superior mesenteric vein (SMV) and the splenic vein meet. From there, the portal vein travels up and to the right, behind the hepatic artery, until it reaches the liver.

Upon entering the hilum of the liver, the portal vein divides into two branches. These are the left portal vein and the right portal vein. These branches further divide into additional branches that travel to different parts of the liver.

The most common anatomical variation is trifurcation of the portal vein. This is when the portal vein divides into three branches, instead of two, as it enters the hilum. In this case, the branches are:
