The gallbladder is a pear-shaped organ located just below the liver in the upper abdomen. The gallbladder stores bile, a fluid made by the liver to digest fat. As food breaks down in the stomach and intestines, bile drains from the gallbladder through a tube called the common bile duct. This tube connects the gallbladder and liver to the first part of the small intestine.

When it spreads to other parts of the body, it is called metastasis. Cells break away from where they originated (the primary tumor) and travel through the lymphatic system or blood.

Metastatic tumors are of the same type as the primary tumor. For example, if the gallbladder has spread to the liver, the cells in the liver are actually gallbladder cells. The disease is metastatic to the gallbladder, not the liver.

Many deaths occur when the tumor migrates from the original tumor and spreads to other tissues and organs. This is called metastatic. This animation shows how cells move from the part of the body where they first formed to other parts of the body.

In stage 0, abnormal cells are found in the mucosa (innermost layer) of the gallbladder wall. These abnormal cells can transform and spread into nearby normal tissues. Stage 0 is also called carcinoma in situ.

In stage 1, it may form in the mucosa (innermost layer) of the gallbladder wall and spread to the muscle layer of the gallbladder wall.

Stage 2 is divided into stage IIA and stage IIB depending on which part of the gallbladder it has spread to.

Gallbladder disease is classified into stages depending on the treatment method. There are two treatment groups:

Relapse refers to recurrence (occurring again) even after treatment. Gallstones may reappear in the gallbladder or in other parts of the body.

Metastases are spread from the primary site (where the metastases started) to other parts of the body. Metastatic gallbladder can spread to surrounding tissues, organs, throughout the abdominal cavity, or to distant parts of the body.

Different types of treatments are available to gallbladder patients. Some treatments are standard (currently used treatments) and some are being tested in clinical trials. Treatment clinical trials are studies aimed at improving current treatments or obtaining information about new treatments for patients. If clinical trials demonstrate that a new treatment is better than the standard treatment, the new treatment may become the standard treatment. Patients may want to consider participating in a clinical trial. Some clinical trials are only open to patients who have not started treatment.

Gallbladder disease can be treated with a cholecystectomy, an operation to remove the gallbladder and some of the surrounding tissue. Nearby lymph nodes may also be removed. Laparoscopy is sometimes used to guide gallbladder surgery. The laparoscope is attached to a video camera and is inserted through an incision (port) in the abdomen. To perform surgery, surgical instruments are inserted through different ports. Tissue surrounding the port site may also be removed due to the risk of gallbladder cells spreading into these ports.

If the lesion has spread and cannot be removed, the following types of palliative surgery may relieve symptoms:

Radiation therapy is a treatment that uses high-energy X-rays or other types of radiation to kill cells or prevent their growth. External radiation therapy uses a machine outside the body to direct radiation toward parts of the body.

Chemotherapy is a treatment that uses drugs to stop cell growth by killing them or stopping them from dividing. When chemotherapy is taken by mouth or injected into a vein or muscle, the drug enters the bloodstream and can reach cells throughout the body (systemic chemotherapy).

Bile Duct Cancer

Targeted therapy is a treatment that uses drugs or other substances to find and attack specific cells. The following targeted treatments are being studied for patients whose gallbladder disease is locally advanced and cannot be removed surgically or has spread to other parts of the body:

Immunotherapy is a treatment that uses the patient’s immune system to fight back. Substances made in the body or created in the laboratory are used to strengthen, direct, or restore the body’s natural defenses against .

Immune checkpoint inhibitors. Checkpoint proteins, such as PD-L1 on tumor cells and PD-1 on T cells, help suppress immune responses. The combination of PD-L1 and PD-1 prevents T cells from killing tumor cells in the body (left panel). Blocking the binding of PD-L1 to PD-1 using an immune checkpoint inhibitor (anti-PD-L1 or anti-PD-1) allows T cells to kill tumor cells (right panel).

What Are Common Gallbladder Problems?

For some patients, participating in a clinical trial may be the best treatment option. Clinical trials are part of the research process. Clinical trials are conducted to determine whether new treatments are safe, effective, or better than standard treatments.

Many of today’s standard treatments are based on early clinical trials. Patients participating in clinical trials may receive standard treatments or may be among the first to receive new treatments.

Patients participating in clinical trials also help improve future treatments. Even if clinical trials don’t lead to effective new treatments, they often help answer important questions and advance research.

Some clinical trials include only patients who have not yet received treatment. Other trials test treatments in patients who do not improve. There are also clinical trials testing new ways to prevent recurrences or reduce the side effects of treatment.
