Jan 9 () – The American Academy of Pediatrics (AAP) on Monday recommended the use of weight-loss drugs in children over 12 years of age to treat obesity. Obesity affects approximately 14.4 million children and youth in the United States and can lead to serious health complications.

“I think they’re important because there are a lot of misconceptions about what exactly causes obesity, and even on the part of doctors there are some unintentional biases about childhood obesity,” said Dr. Marc Michalsky of Nationwide Children’s Hospital in Columbus, Ohio. co-author of the guidelines.

The panel of experts said treatments such as diet pills orlistat, Novo Nordisk’s ( NOVOb.CO ) semaglutide – an injectable diabetes treatment for weight loss under the brand name Wegovy – and the older generic diabetes drug metformin can be given through health behavior and lifestyle changes.

Metformin has been used according to the instructions to achieve weight loss in children. Of the 27 randomized metformin weight loss studies evaluated by the guideline panel, 74% showed a positive effect of the drug.

“In particular, children with immediate and life-threatening co-morbidities, the elderly and those with more severe obesity may need additional treatment options,” the team said.

The threshold for childhood obesity is a body mass index (BMI) – the ratio of weight to height – above the 95th percentile in children of the same age and sex. Severe obesity is defined as a BMI that is 20% higher than the 95th percentile cutoff.

GlaxoSmithKline’s ( GSK.L ) orlistat is approved by the U.S. Food and Drug Administration for the long-term treatment of obesity in children aged 12 and older. Phentermine, an appetite suppressant, is approved for people 16 years of age and older.

Wegovy received US approval last month for chronic weight management in children 12 years of age and older. Novo Nordisk’s diabetes drug Saxenda (liraglutide) has also been approved by the FDA for the treatment of obesity in adolescents aged 12 years and older.

The FDA also approved Vivus’ Qsymia (phentermine and topiramate extended-release capsules) as an additional indication for chronic weight management in obese patients over 12 years of age.

The guideline panel noted that many of the trials it reviewed excluded children with mental health problems, physical activity limitations or the use of certain medications.

“In clinical practice, these children often need the most support in obesity management,” the authors said.

The AAP said there is currently not enough evidence to recommend using these drugs for children ages 2 to 12.

They also include recommendations to diagnose obesity annually in children over the age of 6 through BMI checks and practices such as Motivational Interviewing.

The group also recommends metabolic and bariatric surgery as a treatment for severely obese adolescents.

Intensive health behavior and lifestyle counseling for children and parents for 3 to 12 months is an effective treatment for childhood obesity, but is “challenging to implement and not universally available,” the AAP said.
